How to Schedule with Medicaid Insurance
At Louisiana Pain Specialists, we strive to make scheduling easy for Medicaid patients. Depending on your location, we require specific documentation from your referring provider, including referrals, office notes, and imaging. Please see the guidelines below for more details on what’s needed to schedule your appointment.
For Patients with Medicaid, We Accept the Following Types of Medicaid:
- Bayou Health
Louisiana Healthcare Connections - Bayou Health
United Health Community Plan - Bayou Health
AmeriHealth Caritas*
*Only with Amerihealth Authorization for Tier 3 program*
For Patients Within the Surrounding Greater New Orleans Area/Northshore:
To be scheduled with Medicaid insurance, please have your referring provider fax over:
- A referral
- 2 sets of office notes
- Recent imaging (x-rays, MRI, AND/OR CT) for conditions being treated within the past 2 years.** These items are required to be scheduled.
For patients Outside the Surrounding Greater New Orleans Region:
To be scheduled with Medicaid insurance, please have your referring provider fax over:
- A referral
- 2 sets of office notes
- Advanced imaging (MRI or CT) for conditions being treated within the past 2 years.* * Please note that only an x-ray for the conditions being treated is not sufficient. These items are a requirement to be scheduled.
